Abstract Background Adenomatoid tumor is a very rare benign neoplasm of mesothelial origin affecting mainly female and male genital tracts. The diagnosis challenging as this mimics many differential diagnoses. current literature offers only some case reports short series adenomatoid tumors. Case presentation A 47-year-old patient with unremarkable medical history presented for chronic mild pain the right testis evolving months. physical examination shows palpable intrascrotal nodule 10 mm in greatest diameter. was painful, mobile firm consistency. laboratory investigations were within normal limits, scrotal ultrasonography showed well-circumscribed predominantly hyperechoic epididymal head heterogeneous echostructure. Excisional biopsy lesion performed histopathological analysis microcystic trabecular architecture made small interconnected tubules cysts lined by flattened cells prominent vacuolization thread-like bridging strands, consistent an tumor. postoperative course uneventful discharged. Four months after surgical treatment, has no sign disease. Conclusion Testicular tumors are uncommon neoplasms diagnostic challenge. arising epididymis managed excisional testis-sparing surgery avoiding unnecessary orchidectomy.
